Sistema TS
Report paid Italian invoices to the Sistema Tessera Sanitaria for the patient's pre-filled tax return.
This page is for practices in Italy. A practice in another country does not see it, and the API refuses Sistema TS requests from one.
Settings → Sistema TS connects an Italian practice to the Sistema Tessera Sanitaria healthcare-expense service. Every paid invoice is reported once unless you mark it not reported, each refund of its payments is reported as a refund of that invoice, and a voided invoice is cancelled, so the practice meets its yearly obligation without exporting a file.
Connect
Owners and managers enter the practice's Sistema TS username (the codice fiscale the service knows the user by), password and pincode, pick Test or Production, and, when the documents belong to a proprietario other than the user, that person's codice fiscale. The credentials are encrypted at rest and never shown again; Replace credentials starts a new set. Documents are keyed on the partita IVA the invoice was issued with — the legal entity's, from Settings → Practice details → Legal entities, as it stood at issue — the invoice number and its date, and the dispositivo (1 unless the practice issues from several devices). Changing the partita IVA later never reports an earlier invoice again under the new one.
What is reported
Each night, every issued Italian invoice whose charges are fully settled by
payments becomes a submission, unless it is not reported or its code is still
to follow (see what each invoice tells Sistema TS):
the payment date, the codice fiscale the invoice holds, the amounts by
expense type (SR for treatment, AD for goods) with the VAT nature or
rate, and whether every payment was traceable.
Write-offs, discounts and reversed payments settle nothing STS can report.
Click Report paid invoices now to run that pass immediately.
Once an invoice has been acquired, each refund of the payments that settled
it is reported as a refund of that invoice, for the refunded amount, on the
refund's date and with its own traceability. Each refund is a document of
its own, numbered after the invoice (12/2026/R1, /R2, …), so two refunds
of one invoice — even equal, on the same day — stay distinct at the service;
a refund keeps its number for good. Its amounts continue across the
invoice's expense buckets from where the earlier refunds stopped. A refund
of a payment that settled several invoices is split across them in the order
the payment was applied, so every cent is reported once. An invoice that was
paid and then credited before it was ever reported is still reported as paid,
and its refunds follow. A credit note alone — or money left as credit on the
account — reports nothing. A synced invoice that its practice-management
system voids is cancelled; a credited invoice is never cancelled, because its
refunds are reported instead. A refund that is reversed on the ledger after
the service acquired it is cancelled under the number the service holds.
What each invoice tells Sistema TS
When you issue an invoice at an Italian site, the dialog asks what Sistema TS is told about it:
| Choice | What happens |
|---|---|
| Report with the codice fiscale | The expense is reported with the patient's codice fiscale, which is printed on the invoice too. The patient needs a codice fiscale on file. |
| The patient objects | The patient has exercised their right to object: the expense is reported without the code, flagged as opposed. A codice fiscale on file is still printed on the invoice. |
| Not reported | Nothing is sent, for the reason you give (up to 200 characters) — for example a visitor who has no Italian codice fiscale. A codice fiscale on file is still printed on the invoice. |
| Code to follow | The code isn't on file yet. The invoice is issued but nothing is sent: it waits behind a Sistema TS: codice fiscale to follow task on the patient until you resolve it. |
The dialog starts on The patient objects when the patient's Objects to Sistema TS reporting is on, otherwise on Report with the codice fiscale when they have a codice fiscale, otherwise on Code to follow. A missing code never stops you issuing an invoice.
The choice is kept on the invoice from the day it is issued, and the patient's Account shows it beside the invoice — the choice, not whether Sistema TS has received the document yet; the service's answers are listed on this page. Changing the patient's codice fiscale or objection afterwards never changes an issued invoice or what is reported for it, and the invoice's PDF, if it is made again, prints the code the invoice holds. A credit note carries its invoice's choice.
Resolve a code to follow
To report the invoice with the code, first record the patient's codice fiscale.
Open the invoice on the patient's Account and choose Resolve.
Report it with the codice fiscale now on file, record that the patient objects, or mark it not reported with a reason.
An invoice is resolved once, and its credit notes are resolved with it; their PDFs are made again so they print the code the invoice now holds. Once it is paid, an invoice resolved to report or object goes in the next pass. The task closes once none of the patient's invoices is still waiting for a code.
Reissue an invoice issued without a partita IVA
Sistema TS keys every document on the partita IVA it was issued with, so an invoice issued while the legal entity had none can never be reported, even under a partita IVA set later. Its task, Sistema TS: invoice issued without a partita IVA, asks you to reissue it:
Set the legal entity's partita IVA (VAT number) under Settings → Practice details → Legal entities (see Legal entities).
Open the invoice on the patient's Account and choose Reissue.
In one step Smileline issues a credit note for the old invoice, posts its charges again, issues a new invoice under the partita IVA and moves the payments that settled the old invoice onto the new one, so the new invoice is reported with the original payment dates and methods. The new invoice tells Sistema TS what the old one did — the same choice, code and reason — so a patient's objection stays an objection. The old invoice is marked not reported, keeps the code it printed, and the task closes once none of the patient's invoices still lacks a partita IVA.
An erased patient's kept invoice that lacks a codice fiscale or a partita IVA opens no task: the gap waits for the owner in the review queue, one item per invoice, naming no one, because the erased patient's record can no longer be changed or reissued. Once the owner marks an invoice's item as handled, the daily check leaves it handled.
Reissue an invoice whose codice fiscale was refused
The codice fiscale an invoice reports is the one it was issued with. When Sistema TS refuses a document because of that code, correct the patient's codice fiscale, then open the invoice on the patient's Account and choose Reissue. Smileline credits the invoice, issues it again reporting the corrected code and moves its payments onto the new invoice, keeping their dates and methods; the old one is marked not reported. This is offered only for a document Sistema TS refused and never acquired: an acquired document is never sent twice. A copy of the old invoice still waiting to be sent — queued again after you replaced the credentials, say — is withdrawn and never goes. While an attempt to send it is waiting to retry after an outage, Reissue waits: Sistema TS may have received it, so reissue once that attempt has its answer.
Reissue takes an invoice whose charges are treatments, manual charges or membership fees; the stamp duty is added to the new invoice again by itself. Any other invoice is put right by hand, with a credit note and a new invoice.
Traceable payments
Sistema TS asks whether each expense was paid in a traceable way. Each payment method says so with its Traceable payment switch under Settings → Payment methods (see Payment methods), which starts on for every kind except cash and other. An invoice is reported as paid traceably only when every payment that settled it was made by a traceable method, and each refund is reported with its own method's setting.
Attempts and answers
Each submission is carried within the minute. The service's answer is kept in full; an acquired document shows its protocollo, a refused one shows the service's codes and opens a review task, assigned to nobody, and an outage or a replaced credential set parks the attempt and retries it with a growing pause. On a refused document, Open the review tasks opens Team tasks in To do, where the task waits for whoever takes it on (owners, managers, agency staff and the front desk). Nothing is dropped and no attempt limit applies. A refused document is not forgotten: correct what the service objected to — the proprietario and codes on this page — and the next pass reports it again as a new submission. The patient's codice fiscale goes as the invoice holds it, so correcting it on the patient doesn't change an invoice already issued with it: reissue that invoice instead.
Documents reported
Documents reported lists every submission, newest first. Narrow it with From and To (the practice's days, both included), Kind (inserimento, variazione, rimborso or cancellazione) and State, the latest attempt's state, or Queued for a document no attempt has carried yet. Click Load more to continue the list; it runs to the practice's first document.
Open Attempts and related documents under a document to see every attempt, each with its state, its time, the protocollo or the service's codes, and why it parked. Below the attempts, This document's submissions lists every submission of the same invoice, oldest first: the original, each refund, each cancellation, and the submission each one refunds or cancels. The list never shows the fiscal payload itself.
If the connection or the list can't be read, the page says so with Retry. A failed read never shows as an empty list; when a refresh fails, what was already shown stays, marked as possibly out of date.
The yearly deadline is 31 January for the previous year's documents (with a seven-day window for corrections). Connect before the year ends so every paid invoice has been acquired; the list on this page is the practice's evidence.
